強化學習是人工智慧領域重要的一支,這種學習方法的精神是仿效人類在探索未知領域時的一種策略行為,因此廣泛應用在未知環境的開拓探索,包括涉及人體的醫療決策。精準醫學之個人化醫療強調根據實際臨床觀察數據以估計最佳動態,在適當的病情、適當的時間、適當的支持下,採取適當的醫療措施,強化學習的動態適應性探索能力合適於精準醫學之治療方案研究。基於強化學習技術,本研究著眼於癌症化療療程劑量之控制,其新穎處在於獎勵函數的設計,取代前人引用之簡單常數開關函數獎勵設計,改採更能體現醫病治療行為的動態權衡函數設計,合理的獎勵函數設計可以使智慧體在預測化療劑量更貼切滿足治療的有效性,從而更準確地找到癌症患者成功康復的

施藥軌跡。本研究以施藥的穩定性為基本理提出兩個重要的性質作為評估藥劑預測的性能,分別是預測藥劑量與醫師實際藥劑量的接近度,所謂的信賴度,以及前後治療週期藥劑量不可驟然激烈變化的藥劑慣性。本研究提出的獎勵函數不僅根據學理以數學闡述獎勵的獲取,還結合了實際臨床數據收集模型資料,加以訓練建立模型,配合離線強化學習之保守Q學習的離線策略優化演算法,建立完整之癌症化療療程劑量之滾動預測骨幹模型。實驗顯示在上述兩個重要性質的表現優於過往文獻模型,敏感性分析亦顯示相同傾向的成果。另外,實驗亦證實隨著所累積的訓練樣本數增加,其藥劑預測的精度、信賴度、以及藥劑慣性也都呈現相應的改善。據此歸結所提出的獎勵函數能夠
