研究背景:飲酒所造成的健康危害已成為全球重要的公衛問題,且其亦為死亡的第三大危險因子,酒精使用與70多種疾病有因果關係,可能會造成龐大的疾病負擔及經濟負擔。而酒精使用疾患不僅會對患者生理及心理造成影響,甚至會引起家庭及社會的問題,過去研究指出酒精使用疾患因個案就醫習慣及常被診斷為心理疾患而被忽略,病患多因無病識感且酒癮治療並未納入健保給付而降低病患的就醫意願而造成酒癮問題愈來愈惡化。衛生福利部心理與口腔衛生司於2006年開辦「酒癮治療費用補助方案」,期望能提升酒癮個案主動接受治療的意願及治療的普遍性。然而,2019年全國參與本計畫個案僅2,113位,且台灣酒癮盛行率仍有逐年上升的趨勢。因此,

本研究欲透過個案相關檔案分析了解參與此計畫個案特性及計畫效果,並對「酒癮治療費用補助方案」進行成本效益評估,以評估此計畫是否有達到其實施目的與價值。由於僅各縣市衛生局有個案基本資料及相關評估紀錄等資料可以進行較精確的評估,因此,本研究使用「新北市飲酒減量醫療戒治服務計畫」之紙本資料自行建檔,針對酒癮治療補助方案進行效果評估及成本效益分析。研究方法:以單組前後測研究設計,針對新北市飲酒減量醫療戒治服務計畫個案的完治狀況、酒癮程度等指標進行效果評估;並以社會觀點,以成本效益分析評估新北市政府衛生局飲酒減量醫療戒治服務計畫之成本、所帶來的效益、淨效益及其益本比,成本包括實施計畫之直接成本及間接成本,

效益則包括此計畫會帶來之直接效益及間接效益;最後,以馬可夫鏈蒙地卡羅法模擬當參與計畫總人數為5,000人、10,000人、28,850人時的完成治療率之後驗分佈,以計算出不同模擬人數分別在計畫之一年效應、兩年效應及三年效應下,其成本效益結果。 研究結果:整體來看,參與新北市飲酒減量醫療戒治服務計畫之完成率為32.3%,完成治療平均時間為1.43年。在控制其他因素下,其他單位轉介、於A院所就醫者完成治療的機率顯著較高,而接受心理治療9次以上、接受17次以上個案管理服務,其完成治療的機率較低。在控制其他控制變項後,不論是以1年、2年、3年為切點,新北市飲酒減量醫療戒治服務計畫的介入皆會使個案酒癮程

度(AUDIT)顯著下降,但不同時間切點的AUDIT分數降幅度無太大差異。分析計畫三年成效,51歲以上、轉介來源為非精神科或成癮防治科門診或病房或其他單位的個案,在介入後AUDIT分數顯著下降,而接受個案管理者之酒癮程度則無顯著影響。由社會的觀點來看,評估新北市飲酒減量醫療戒治服務之一年、兩年與三年的淨效益分別為88,189,261元、51,737,874元、15,655,002元,益本比則分別為16.24、7.72、3.56。益本比隨著個案參與計劃時間愈長而降低,但不論計畫為一年、兩年或三年皆符合成本效益。針對計畫成本、完成治療率、折現率等不確定因素進行敏感度分析,皆未改變原來結果。進一步以

馬可夫鏈蒙地卡羅法模擬當參與計畫總人數為5,000人、10,000人、28,850人時,完成治療率之後驗分佈,可發現治療率與成本效益比雖略下降,但仍具成本效益。結論與建議:本研究發現新北市飲酒減量醫療戒治服務計畫不僅可有效改善酒癮個案的酒癮程度,且具有成本效益,值得繼續並擴大推動,而目前參與新北市飲酒減量醫療戒治計畫之個案占全國約25%,建議衛福部以新北市為標竿鼓勵其他縣市積極推動此計畫。但目前計畫完成治療率僅約三成且各院所收案人及完成治療人數有落差,建議應進一步探討其原因,並持續精進計畫之成效。
