藝術療法介入對社區失智據點老人認知功能與憂鬱情緒之成效探討

台灣進入高齡社會,失智議題為當前社會的重要課題,認知功能退化與情緒障礙是老年失智者首要面對的主要問題,然而目前針對失智症藥物治療效果有限。本研究主要目的乃在探討藝術療法介入對於社區失智據點失智老人認知功能、憂鬱情緒改善之成效,進而比較分析接受藝術療法介入實驗組與未接受介入對照組之差異。本研究方法採類實驗設計,以方便取樣方式選取高雄市3個行政區社區失智據點65歲以上失智老人為研究對象,採隨機分派分為實驗組(n=32)和對照組(n=30),實驗組接受十二週,每週一次,每次90分鐘的藝術療法活動,對照組則維持參與據點原先日常活動。兩組採用簡易智能評估量表(MMSE)、康乃爾失智者憂鬱量表(CSDD

)作為評估工具,分別於研究介入前及介入12週後進行資料收集。研究結果顯示在認知功能(MMSE)方面,兩組達顯著差異( t = 2.562,p = .0104 );在憂鬱情緒(CSDD)方面,兩組達顯著差異( t = -3.854,p < .001 )。再經由多元迴歸統計分析,在控制實驗組別、年齡、性別、教育程度、宗教信仰、婚姻狀況、慣用語言、運動習慣、參與活動期間、慢性病數等變項後,顯示實驗組別對於認知功能的影響具有解釋力(β= -1.989,p = .009),迴歸模式解釋力為33.8%;憂鬱情緒的影響具有解釋力(β= 3.669,p < .001),迴歸模式解釋力為47.7%。 社區

失智據點老人在藝術療法活動介入後,其認知功能及憂鬱情緒較介入前有顯著改善,對於提升認知功能及情緒穩定具有正面改善效益。因此,藝術療法可以應用於失智老人的長期照護,帶來更優質的照顧活動規劃,達到非藥物輔助療法的效果。

In the spring of 2012, the Department of Architecture at The University of Hong Kong conducted a series of Research and Design (R&D) studios within its Master of Architecture Program. One of the R&D topics, led by Mr. Anderson Lee, was to study and examine the potential role and developme

nt of a historical building in the heart of Hong Kong: The Old Wan Chai Police Station and Married Quarters. The students were asked to address issues ranging from public space in contemporary cities to heritage conservation, from design and building technology to sustainable planning. This book pre

sents the recorded research and subsequent design proposals; however, they are by no means the final conclusion on such a complex topic. In the studio, which lasted only 12 weeks, the students tried to identify: issues of scales that inform and affect architectural decision-making; the role of stake

holders pertaining to development; the urban and economic forces that shape the morphology of the urban fabrics; a way to integrate new programmatic elements within the existing urban fabrics; and to design a mixed-used project on a constrained site with a given formal and tectonic logic. The Old Wa

n Chai Police Station and Married Quarters remains largely uninhabited and its future is yet to be determined. Under the current political climate in Hong Kong, it is almost certain that this piece of colonial past will sit idle for a few more years...
