OTOF基因聽神經病變與耳蝸神經發育不良兒童人工耳蝸術後之聽語表現

研究目的:由於國外人工耳蝸對內耳及聽神經病灶之效果均已多有研究,但國內多限於內耳疾病,因此對於聽神經病灶(聽神經病變(ANSD)及耳蝸神經發育不良(CND)加以研究,並比較其臨床客觀評估效益與家長意見之一致性與相關性。本研究旨在探討OTOF、OPA1及WFS1基因ANSD兒童與CND兒童使用人工耳蝸術前及術後聽語表現,並比較收案兒童術後家長意見回饋差異及臨床主觀評估和家長意見之相關性。透過本研究結果供臨床醫事人員提供家長人工耳蝸術前諮詢與術後復健相關建議,以利家長預期個案術後聽語發展可能性。材料與方法:採回溯性分析2015年至2021年間,經基因檢測、聽性腦幹反應檢查(ABR)、變頻耳聲傳射

檢查(DPOAEs)、磁振造影(MRI)、電腦斷層(CT)與臨床聽力學表徵確診為OTOF、OPA1及WFS1之ANSD與CND兒童,並於台大醫院、台中慈濟醫院接受人工耳蝸手術之兒童,依據影像學上耳蝸神經、前庭神經及內耳發育情形,以Govaerts分類其嚴重度(Type I、Type IIa、Type IIb),透過兒童聽覺表現分類量表(CAP)、語音清晰度量表(SIR)、口語發展度量表(MSLDS)之臨床評估,輔以術後家長主觀意見問卷分析其術後成效,追蹤兩組患者術後聽語表現。本研究排除追蹤時長少於一年之個案,比較個案術後一年、兩年之聽語表現。結果:本研究共收案35位兒童,統計結果顯示OTOF-

ANSD組與CND組兒童術後一年的CAP進步幅度皆優於術後第二年,且前者術後一年的SIR進步幅度亦優於術後第二年。OTOF-ANSD組兒童術後家長意見表現優於CND組兒童,學齡前期CND兒童家長意見問卷回饋顯示Type IIb優於Type IIa和Type I兒童。臨床主觀評估得分與家長意見回饋具中高度相關性。結論:本研究透過對OTOF、OPA1及WFS1基因之聽神經病變與CND個案植入CI前後之成效進行系統性分析整理,以利供未來相關研究iv做為參考資訊,顯示OTOF-ANSD 組兒童植入人工耳蝸術後成效顯著優於 CND 組兒童,且耳蝸前庭神經及內耳發育不全的嚴重度可以預測 CND 兒童接受人

工耳蝸之術後成效。而家長意見問卷除了可以反應主要照顧者對於兒童術後成效的主觀感受之外,臨床人員亦可透過家長意見問卷中獲得資訊加以提升臨床主觀評估信度。
