肥胖造成肝臟從良性的脂肪肝轉變成具炎症反應的非酒精性脂肪性肝炎(nonalcoholic steatohepatitis, NASH),為現今重要研究議題。透過活化肝細胞的ASK-1 (apoptosis signal kinase 1)調節相關激酶:p38和JNK (c-jun n-terminal kinase)被認為與NASH進展有關,其中TNFAIP3 (tumer necrosis factor alpha-induced protein 3)為ASK-1負調控的重要因子,在基因缺損TNFAIP3的小鼠表現出明顯NASH性狀可證實。但此機轉被運用到臨床試驗及其動物模式文獻報告仍有限

目的:本研究旨於冠狀動脈繞道術後糖尿病患者其自我管理與生活品質之相關性探討。背景:隨著物質生活水準的改善、公共衛生的進步及醫療科技的突飛猛進,人類壽命已逐漸延長,疾病型態亦由過去的急性傳染病轉變為現今以慢性疾病為主,以生活在台灣地區的居民而言,由於飲食型態的改變、都市化生活造成人們日常運動量減少,肥胖人口增加,使得心血管疾病及糖尿病的盛行率也因而增高,不僅造成醫療費用負擔,更衝擊病患的生活品質,然而糖尿病又是需要終身自我管理的慢性病,而現階段少有針對冠狀動脈繞道術後糖尿病患者其自我管理和生活品質之研究,故執行本研究希望藉由了解其手術後之自我管理與生活品質,以期提供術後衛教更多的資訊。方法:本
研究採橫斷性研究方式,以立意取樣方式收集資料,於北部兩家醫學中心及一家區域教學醫院,採問卷調查方式收集資料,共166人參與研究。研究工具包括自我管理問卷、SF-36臺灣版生活品質問卷和基本資料問卷。研究資料以SPSS 18.0電腦套裝軟體進行分析,統計分析包括百分比、平均數、標準差、相關係數和逐步迴歸分析。結果:本研究結果顯示自我管理與生活品質有顯著相關,推論統計發現(1)自我管理平均為25.10±.15.16,表示患者的健康自我管理屬於中上。(2)性別、教育、婚姻、手術前職業、目前有無工作、經濟來源、主要照顧者、喝酒史、運動狀況、糖尿病病史、體重評估、冠狀動脈手術時間、共病數與自我管理呈顯著
相關。(3) 生活品質次量表得分最高是「軀體疼痛」,得分最低是「一般健康」。(4)性別、教育、婚姻、手術前職業、目前有無工作、經濟來源、主要照顧者、抽煙史、喝酒史、運動狀況、糖尿病病史、糖尿病合併症、藥物種類、體重評估、冠狀動脈手術時間、手術後不適症、共病數與生活品質中的某些層面呈顯著相關 (5)經逐步迴歸分析結果發現自覺對疾病的了解、主要照顧者、年齡、婚姻可解釋自我管理29.7%之變異量,最重要之預測因子為自覺對疾病的了解。(6)經逐步迴歸分析結果發現糖尿病合併症、主要照顧者、規律運動、接受個別糖尿病指導、手術後不適症、糖尿病病史、性別、自我管理,可解釋生理生活品質41.2%之變異量,最重要
之預測因子為糖尿病合併症。自我管理、接受個別糖尿病指導、糖尿病合併症、生活環境滿意度,可解釋心理生活品質40.2%之變異量,最重要之預測因子為自我管理。(7)進一步分析基本屬性之主要照顧者可透過自我管理影響的中介作用正向影響生活品質,結果有17.7%的解釋變異量。結論:本研究結果有助於作為冠狀動脈繞道術後糖尿病患照護實務的實証基礎,且支持冠狀動脈繞道術後糖尿病患之照護應強調促進病人的自我管理,以提升生活品質。並提供臨床醫療團隊建立冠狀動脈繞道術後糖尿病患自我管理規劃設計,作為病患照護計畫的基礎,設計真正有效且符合病患日常生活簡易執行的自我管理計畫,使冠狀動脈繞道術後糖尿病患能運用現有的資源,與
