研究背景:衰弱症候群(Frailty Syndrome)為老人醫學領域中一門新興的概念,與老化(aging)和失能(disability)高度相關,但意義並不全然相同,進程可分為衰弱前期(Pre-Frail)與衰弱期(Frailty),其中衰弱前期的早期偵測,對衰弱的預防至為重要;儘管學者已普遍認同衰弱對銀髮族群之重要性,及其所造成的醫療資源耗費,不過臨床上評估方法尚未統一,內容亦未見建立標準,且大多採用問卷回顧與人工測量的方式,例如 Fried 衰弱判別指標(Fried Frailty Criteria),雖然廣受學者們採用,但卻存在著耗時費力,易生誤差的缺點,因此極需較客觀、快速、精確、

標準化的工具。若能藉由一日常生活中可便利使用的裝置,隨時去評量或檢測衰弱的症狀發展,應可收到早期發現、察覺並復健的效果,平價而功能完整的智慧型手機(Smartphone)應是一相當合適的應用標的。目的:本研究之目的為以智慧型手機為基礎,建立一衰弱檢測與早期介入之個人化隨身健康助理系統。方法:首先,依據 Fried Frailty Criteria,設計搭載於智慧型手機上的衰弱檢測與早期介入軟體(APP),用以檢測自身衰弱的徵狀,及提供個人化的早期介入對策(每日衛教提醒與指導、每日活動記錄與分析、雲端資料庫的同步與備份)。其次,招募高齡長者參加檢測,並使用 Spearman’s correlat

ion 相關性統計法與組內相關係數(Intraclass correlation coefficient,ICC)分析本系統與傳統方法對於衰弱因子的鑑別結果,以驗證本系統的衰弱檢測準確性(效度);然後,針對衰弱前期個案所表現出的衰弱因子,利用智慧型手機施以 12 周的介入後,以配對 t 檢定(paired t-test)與卡方檢定(X2 test)分析衰弱因子介入前後的變化,探討本系統是否可對早期衰弱的徵狀產生復健效果。統計顯著水準設定為 α = 0.05。結果:收測過程中一共接觸了 48 位符合條件者,但其中有 11 位完全不願意使用智慧型手機、15 位雖有適合手機但是拒絕參與研究。在 22

位受試長者中,屬於衰弱者與衰弱前期者各有一位,其餘均未出現衰弱徵象,且傳統人工檢測與智慧型手機檢測的結果並無二致。兩檢測方式對於一周身體活動量(r = 0.747,ICC = 0.881)與自選行走速度(r = 0.800,ICC = 0.743)的測試結果均呈現高度相關;然而,由於衰弱前期的收案數較少,僅能透過觀察得到本系統對於衰弱因子的改善具有潛力,未達到足以分析的案量。結論:本研究建立的衰弱篩檢系統,可以取代傳統的人工測量或問卷調查,快速且準確地瞭解老年人的衰弱狀況。同時,電子化的量測,可提供量化的數據,定義衰弱的嚴重程度或是惡化速度,做為衰弱介入策略的依據,進而採行不同積極程度的介入

策略,對有早期衰弱症狀的受試者提供個人化早期介入方案,提升其身體功能與活動能力,助其恢復健康老化的狀態,將有效避免醫療資源的浪費,妥善運用社會福利,降低整體社會成本的支出。未來可望更進一步結合無線行動網路傳輸技術,便能快速精確地建立起本地完整的衰弱資料庫,有利更全面的衰弱症候群篩檢,對衰弱的早期檢測與復健具備良好的應用潛力。
